Albendazole exerts its therapeutic effects primarily through its active metabolite, albendazole sulfoxide. This metabolite selectively disrupts the microtubule structures within the cells of parasites, particularly those in the intestinal cells and cortical layers. By binding to the β-tubulin subunits in the parasite's microtubules, it inhibits microtubule polymerization, thereby interfering with the intracellular transport and nutrient absorption of the parasites. Additionally, albendazole reduces the uptake of glucose by parasites, depleting their glycogen reserves, which ultimately leads to energy exhaustion, paralysis, and death of the parasites.
At higher concentrations, albendazole can also inhibit key metabolic enzymes, such as malate dehydrogenase and fumarate reductase, disrupting the metabolic pathways of parasites and further reducing ATP production. This leads to the inability of parasites to maintain normal physiological functions. This mechanism of action allows albendazole to be effective against a wide range of parasites while having minimal impact on host cells.
